Which Foods Have Lactose? A Practical Guide by Food Type

Processing matters because lactose travels mainly with the watery milk phase: fermentation consumes some of it, cheesemaking removes much of it with whey, and butter making concentrates fat while leaving most milk sugar behind. For lactose intolerance, the dose you eat at one time is usually more informative than whether an ingredient originated from dairy.

Milk, evaporated milk, and milk-based drinks are the clearest sources

Regular cow’s milk contains roughly 12 grams of lactose per cup, making it a useful reference point. Goat and sheep milk also contain lactose; switching animal species does not make milk lactose-free. Chocolate milk, flavored milk, milkshakes, and beverages made with substantial milk can provide a similar or larger lactose dose depending on serving size. Evaporated and condensed milks concentrate milk solids, so a small amount can contribute meaningful lactose. Lactose-free cow’s milk is different: manufacturers add lactase or otherwise reduce lactose, while the milk proteins and most other nutrients remain.

Yogurt is more complicated than the word “dairy” suggests

Yogurt starts with milk, so it is not naturally lactose-free. During fermentation, bacteria use some lactose, and live cultures can provide beta-galactosidase activity that helps break down lactose in the digestive tract. This is one reason many people with lactose maldigestion tolerate yogurt better than an equivalent amount of milk. Greek-style or strained yogurt may contain less lactose per serving because whey is removed, but recipes differ and sweetened products can vary substantially. If you need a predictable very-low-lactose option rather than simply “often better tolerated,” choose a product explicitly labeled lactose-free and check the serving you actually eat.

Cheese ranges from fresh and milky to naturally very low in lactose

During cheesemaking, lactose dissolves into the whey that drains away from the curd. Starter cultures then ferment much of what remains. Fresh, moist cheeses can retain more lactose because they contain more whey and undergo less aging. Long-aged hard cheeses such as Parmesan-style cheeses and many mature cheddars generally contain very little. This does not make every cheese identical, and exact amounts vary by manufacturing method. If symptoms are sensitive to small doses, manufacturer information or a lactose-free label gives more certainty than assuming all hard cheese is zero.

Butter and cream contain little lactose, but “little” is not “none”

Cream separates the fat-rich portion of milk, so it contains less lactose per typical serving than milk. Butter goes further: it is mostly milk fat, and most lactose stays in the buttermilk during churning. As a result, ordinary portions of butter usually contribute only a small amount of lactose. That can be useful for many people with lactose intolerance, but it does not make butter dairy-free or safe for a milk-protein allergy. Whey protein concentrates, milk powders, and other dry dairy ingredients can move in the opposite direction by concentrating milk solids, so processed foods deserve an ingredient-label check.

Hidden lactose is mostly a matter of milk-derived ingredients and serving size

Bread, sauces, instant mashed potatoes, soups, chocolate, protein products, and prepared desserts may use milk, whey, milk powder, cream, or other dairy ingredients. Their lactose content depends on the formulation; the mere presence of a dairy-derived ingredient does not reveal the dose. Medication tablets can also use lactose as an excipient, although the amount is usually small. For food, start with the ingredient list and Nutrition Facts, then use manufacturer information when an exact lactose value matters. Labels for milk allergens are designed for allergy, not as a quantitative lactose meter.

Tolerance is individual, so build the diet around dose rather than fear

Lactose intolerance results from limited lactase activity, not from an immune reaction to milk protein. Many people can tolerate some lactose, particularly in smaller portions eaten with other foods, while others need much stricter reduction. Lactase tablets and lactose-free dairy can expand choices. The useful hierarchy is not “dairy versus no dairy,” but higher-lactose foods, lower-lactose foods, and the amount that fits your own tolerance.
